  • Once you are welcomed by our Clinic Front Desk staff, we will collect the required information to complete the registration process. One of our highly trained physical therapists will then take a medical history, perform a thorough physical evaluation and begin treatment. Based on his/her findings, an individual treatment plan will be created. Most patients will receive some type of manual therapy to improve joint and/or soft tissue mobility to restore range of motion and to improve performance of therapeutic exercise and functional activities at home. We will also have discussed with you a personalized home exercise program that is vital to your positive outcome and to help speed your recovery.

  • When you visit one of our clinics for the first time, you will need the following:

    1.) Your prescription for physical therapy from your referring physician
    2.) Your insurance card(s)
    3.) Payment for the applicable co-pay or co-insurance
    4.) Registration Form

  • IPT recommends you wear comfortable clothing to your treatments. If your injury is upper extremity, please wear a t-shirt or tank top. If your injury is lower extremity, shorts may be necessary for your treatment.

  • Your initial visit is typically the longest visit and we recommend you plan for a 1 to 1 ½ hour visit.

  • Indiana Physical Therapy accepts all insurance plans including Medicare, Medicaid, Worker’s Compensation, and Auto accident referrals. All patients are encouraged to check with their insurance company to verify that IPT is a contracted provider.
