Patient FAQ
Are you in network with my insurance?
We are in network for most insurance companies. If you
are unsure call the customer service number located on
your insurance card.
How do I know what my Benefits are?
We verify insurance for each patient as a courtesy. We
can tell you what your insurance company has quoted as
your benefits.
How long will the appointment be?
The first initial visit is the longest. The first time
plan to be here 1 to 1 1/2 hours then visits are approximately
45 minutes to 1 hour long.
Do you accept assignment on Medicare?
Yes we do.
Will you bill my insurance company?
As a courtesy to the patient we do bill the insurance
company.
When am I expected to pay my co-pay?
Office visit co pay’s are expected to be paid per visit.
Do you offer payment plans?
Yes, we do offer different types of payment options with
approval.
Do I have to be referred by a physician?
Yes, you do need to be have a referral. Your referral
can come from a M.D, D.O, Dentist, Chiropractor, Nurse
Practitioner or Podiatrist.
What should I expect at my first appointment?
The therapist will take a history then do a thorough physical
evaluation. Based on their finding, treatment will begin
right away and include a home exercise program.
What should I wear?
Make sure to wear or bring loose fitting clothes. A tank
top if you have an upper extremity problem and a pair
of shorts if it is a lower extremity problem.
