Thank you for choosing Indiana Physical Therapy for your
rehabilitation needs. The information that follows will
give you a better understanding of what you can expect
at your first visit. If you should have any questions,
please feel free to contact one of our nine clinics. Each
member of our staff is here to assist you.
Registration
We hope to make the registration process as convenient
for you as possible. If you prefer to save time before
your first appointment, click on the link below and print
the Patient Registration forms. Please bring the completed
forms with you to your first appointment.
What You Need
When you visit one of our clinics for the first time,
you will need the following:
1.) Your prescription for physical therapy from your referring
physician.
2.) Your insurance card(s).
3.) Payment for the applicable co-pay or co-insurance.
4.) Complete Registration
Form.
What to Expect
Once you are welcomed by our Clinic Front Desk staff,
we will make sure you are registered and we collect the
required information. One of our highly trained physical
therapists will then take a medical history, perform a
thorough physical evaluation and begin treatment on your
first appointment. Based on his/her findings, an individual
treatment plan will be created. Most patients will receive
some type of manual therapy to improve joint and/or soft
tissue mobility to restore range of motion and to improve
performance of therapeutic exercise and functional activities
at home. We will also have discussed with you a personalized
home exercise program that is vital to your positive outcome
and to help speed your recovery.
IPT recommends you wear comfortable clothing to your treatments.
If your injury is upper extremity, please wear a t-shirt
or tank top. If your injury is lower extremity, shorts
may be necessary for your treatment.
Your initial visit is typically the longest visit and
we recommend you plan for a 1 to 1 ½ hour visit.
Insurance
Indiana Physical Therapy is contracted with most commercial
insurance plans and also accepts Medicare, Medicaid, Worker’s
Compensation, and Auto accident referrals. For more insurance
